Incisional Atrial Tachycardia Masquerading As Counter-clockwise Atrial Flutter

چکیده
Forty-four-year-old male patient with surgical closure of ostium secundum type atrial septal defect at the age of 9 years had undergone radiofrequency catheter ablation of the cavotricuspid isthmus because of recurrent drug-resistant typical isthmus-dependent atrial flutter, with bidirectional isthmus block achieved. Only few days later he started to suffer again high-rate tachycardia. An atypical atrial flutter was diagnosed on ECG (Figure 1). This one proved to be drug-resistant as well and recurred promptly after cardioversion.

Atrial tachycardia arising from the right atrial inferoseptum masquerading as common atrial flutter.
Radiofrequency catheter ablation was performed in 2 patients with atrial tachycardia (AT). In both cases the AT originated from the inferoseptal portion of the right atrium, and the cycle length was 210 ms. The surface ECG demonstrated common counterclockwise atrial flutter, probably caused by functional block in the clockwise direction at the cavo-tricuspid isthmus and posterior right atrium w...
متن کاملAtypical atrial flutter originating in the right atrial free wall.
BACKGROUND Data from experimental models of atrial flutter indicate that macro-reentrant circuits may be confined by anatomic and functional barriers remote from the tricuspid annulus-eustachian ridge atrial isthmus. Data characterizing the various forms of atypical atrial flutter in humans are limited. METHODS AND RESULTS In 6 of 160 consecutive patients referred for ablation of counterclock...
متن کاملCatheter ablation of Atrial Incisional Tachycardia mistaken for Atrial flutter
Incisional sustained tachycardias are frequent in patients who have undergone a surgical repair of interatrial defect. A 43-year-old woman with drug refractory, highly symptomatic, persistent atrial tachycardia in the last year, was referred to our unit for catheter ablation. The patient had undergone a cardiac operation for repairing interatrial secundum ostium type defect with a patch five ye...
متن کاملAtrial Tachycardias Occurring Late After Open Heart Surgery
Atrial tachycardias are common after open heart surgery. Most commonly these are macro-reentrant including cavotricuspid isthmus dependent atrial flutter, incisional right atrial flutter and left atrial flutter. Focal atrial tachycardias occur less frequently. The specific type of atrial tachycardia highly depends on the type of surgical incision. Catheter ablation can be very effective, howeve...
متن کاملLower loop reentry as a mechanism of clockwise right atrial flutter.
BACKGROUND Right atrial reentrant tachycardia resulting from lower loop reentry (LLR) around the inferior vena cava (IVC) has been described recently. However, all reported cases of LLR in the literature have negative flutter waves on the inferior surface ECG leads similar to that of counterclockwise typical atrial flutter around the tricuspid annulus (TA). Right atrial flutter with positive fl...
